Basin Hospice:

Basin Hospice provides end-of-life care for individuals with life-limiting illness. Our first priority is the comfort of our patients, but we also are dedicated to the families by providing relief and support.

Patient Care:
Regular visits by nurses and home health aides
Pain and symptom management
Psychosocial evaluation and counseling for patient
Spiritual care and counseling
The coordination of supplies, medications, and medical equipment
Hospice aide services that include bathing, grooming and other personal care tasks.

Family Care:
Respite services for relief and support to caregivers
Bereavement counseling
Spiritual care and counseling for family members
Support from trained Hospice volunteers


Admission to Basin Home Health & Hospice can be made under the direction of a physician and is based upon the patient’s identified care needs, homebound status and type of services required that we can provide directly or through coordination with other organizations.

Once a patient is admitted, the process of developing his or her individualized plan of care begins. We involve the patient, the patient’s caregiver, key professionals, and other staff members in developing the plan of care.

A patient’s plan of care is based upon needs, goals, identified problems, treatments and services, physician order for medications, time frames and a patient’s environment. The plan is designed to increase a patient’s ability to care for themselves.
