Admission to Basin Home Health & Hospice is made under the direction of a physician and is based upon the patient’s identified care needs, homebound status and type of services required.

Once a patient is admitted, the process of developing his or her individualized plan of care begins. We involve the patient, the patient’s caregiver, key professionals and other staff members in developing the care plan.
