COMFORT ~ CARE Hospice is a concept of care. Patients
are often most comfortable in the familiar surroundings of their own homes.
The Hospice Staff of highly qualified nurses. social workers. spiritual counselors and volunteers work in partnership with the patient. family. and their personal physician to make the final period of life comfortable. dignified and peaceful.
Effective control of pain and other symptoms is especially emphasized. HOSPICE SERVICES
- Regular visits by nurses and home health aids.
- Psychosocial evaluation and counseling for patient and family.
- Spiritual care and counseling.
- Respite services for relief and support to caregivers.
- The coordination of supplies. medications and medical equipment.
- The pursuit of available reimbursement sources.
PATIENT CARE: The ultimate Hospice concern is for the patient’s comfort. The Hospice care team works closely with the patient’s family and physician to develop a comprehensive plan of personalized care.
Once the patient’s needs have been assessed. regular visits are scheduled with a professional nurse and homehealth aide. Social workers. spiritual counseling and trained volunteers are available to the patient. Our medical director is a certified home care physician and is available to make home visits and consultations as needed. The nursing staff is available for emergency situations 24 hours a day. 7 days a week.
FAMILY CARE: Support services are provided to the family members and friends of the terminally ill. Bereavement counseling, spiritual counseling, relief for caregivers and support by trained volunteers are virtual elements of hospice
REFERRALS AND ELIGIBILITY: Physicians. family members. health care professionals. insurance providers and clergy may recommend patients to Hospice. Admission to the Hospice program is open to residents of San Juan County who have life-limiting illness in which curative medical intervention is no longer possible or desired.
FUNDING: Hospice services are partially funded by – Medicare. Medicaid or private insurance carriers. Therefore. community support through memorials. grants and contributions enables Hospice to keep up with the evergrowing needs of the terminally ill in our community. Hospice care can be provided in a patient’s home. a nursing home or assisted living facilities. No qualified patient is refused Hospice services based on type of illness. age.sex. race. creed or the ability to pay.
