What can I expect from Patient Care

The ultimate Hospice concern is for the patient’s comfort. The Hospice care team works closely with the patient’s family and physician to develop a comprehensive plan of personalized care. Once the patient’s needs have been assessed. regular visits are scheduled with a professional nurse and home health aide.   Social workers. spiritual counseling and trained volunteers are available to the patient. Our medical director is a certified home care physician and is available to make home visits and consultations as needed. The nursing staff is available for emergency situations 24 hours a day. 7 days a week.
